Grafting

Train-the-Trainer Program

The GRAFTING: Train the Trainer initiative is a dynamic professional development program created to build sustainable therapeutic activity frameworks in memory care environments. Just like grafting a branch onto a living tree, this program equips designated trainers with the tools to transfer vital team-building, communication, and engagement techniques into the hands and hearts of activity professionals. 

The Grafting Method

In horticulture, grafting bonds two living parts to create one stronger, thriving organism. In our program, the trainer is the graft—bridging evidence-based learning with daily activity routines. The activity professional is the host—bringing the insight, heart, and flexibility needed to embed these teachings into a living, breathing schedule.

Trainers will:

Participate in immersive team-building modules

Learn trauma-informed and person-centered techniques 

Gain strategies for adapting sessions to dementia and memory care populations

Master monthly implementation goals using an experiential roadmap

How the Trainer Works with the Activity Professional 

Monthly Planning Meetings

Trainers and activity professionals will meet at the start of each month to select core concepts from that month’s GRAFTING toolkit (e.g., communication, purpose, sensory grounding). 

Hands-On Co-Facilitation

Trainers will model and support implementation of the concepts in real-time, within resident groups or one-on-one engagements.

Observation & Coaching

Trainers provide positive reinforcement, adjustments, and documentation strategies to support consistent delivery by the activity professional.

End-of-Month Reflection

Each month concludes with a shared reflection to identify what “took root,” and determine which “branches” to grow next.
